Mark your calendar for our annual Boxes of Blessing Packing Day! Bring your family, small group, and friends to help us extend the Kingdom of Christ to our city by filling 10,000 boxes of food. These boxes are a wonderful gift to our ministry partners that allows them to minister to their communities in a unique way.
If you have registered in advance but 1) do not feel well on December 4, or 2) have potentially been exposed to COVID-19, we request that you not attend the event.
Due to Covid health concerns, by your participation at Boxes of Blessing on Saturday, 12/4/21, you are agreeing to the liability waiver below.
Liability Waiver:
In consideration of my participation in Boxes of Blessing, I acknowledge the following and release Park Cities Presbyterian Church, Boxes of Blessing, B.C. Williams Bakery Service, Inc. and BCW Warehouse Leasing, LP (the “Releasees”) from all liability as follows:
- I acknowledge the existence of physical risk to me which may result from my physical appearance at the venue and my participation in this activity and that such participation may cause injury or illness to me, including but not limited to Influenza, MRSA, or COVID-19, that may lead to paralysis or death.
- I hereby waive, release and forever discharge the Releasees from any and all claims, liabilities, damages or causes of action, known or unknown and at law or in equity.
Health statements:
- I have not experienced symptoms of fever, fatigue, difficulty in breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or any communicable disease within the last 14 days.
- I have not, nor any member(s) of my household, traveled by sea or by air, internationally within the past 14 days.
- I did not, nor any member of my household, visit any area within the United States that was reported to be highly affected by COVID-19, in the last 14 days.
- I have not been, nor any member(s) of my household, diagnosed to be infected of COVID-19 virus within the last 14 days.
If I feel unwell, I will not participate in Boxes of Blessing.
